REABLEMENT

This service is for individuals providing support for people with barriers preventing them from being discharged home from hospital.  Patients referred to the service shall initially receive contact and support, either at hospital or in the home, from YAWR Services, to remove barriers preventing them from a safe discharge home from hospital.  YAWR will provide or arrange transport to assist patients to be safely discharged home from hospital and provide support at home until the patient feels more able to cope and less likely to be re-admitted to hospital.  The service will be a 5 days per week, quick response service to reduce the number of days the patient remains in a hospital bed when medically ready for discharge. The Service Provider will use a Personal Health Budget approach to purchase items and services that may prevent patients’ safe discharge from hospital home.
